Application Materials

Thank you for your interest in Kurn Hattin Home and School for Children. We welcome your application for admission of your child. When all of the information listed below has been received, the decision will be made whether or not to go to the next step of the application process and schedule an interview. A complete application consists of:

Birth Certificate

Please provide a copy of your child’s birth certificate

Application for Admission

Please fill this out as completely as possible.

Court Order Release

Please fill this out and take it to the courthouse that handled your proceedings.

Health Form A

Please fill out this two page form yourself and send it to us.

Report of Health Evaluation B

Take the single page to your child’s doctor, who will fill it out and return it to us.

Release of Student information A

Fill this out and send to us. We will fax to your child's school for a copy of their records.

Release of Student information B

For your child’s most recent counselor, therapist or psychologist. Take this form to their office so that they may send us an assessment or summary. You may also send this form to forward to the therapist.

A Focus on You

Your child should fill this out. If they can’t do it by themselves, you may write down their answers.

Green Teacher’s Report Form

Contact the Admissions office at (802) 721-6913 or email Terry Andrews to receive a copy of this form. Take this to your child’s school. The classroom teacher should fill it out and send it to Kurn Hattin.

Terry Andrews

As Admissions Coordinator, Terry Andrews helps families across New England explore whether Kurn Hattin is the right fit for their child. She seeks children who can benefit from the Kurn Hattin experience, contribute positively to the community, and meet the school’s admissions criteria.

Having earned her master’s degree in 2013, Terry has been part of the Kurn Hattin community for more than a decade. She brings thoughtful guidance and a steady presence to families navigating important decisions.

Outside of work, Terry loves spending time with her family, especially her three young grandchildren. She also enjoys reading and gardening in the spring and summer months.
